Lee's Summit Pointe Health and Rehab is seeking an Admissions Coordinator for our newly renovated facility. The ideal candidate will be a dynamic self-starter with at least one year of experience in a skilled nursing facility and will have excellent communication and organizational skills. Lee's Summit Pointe is a small 32 bed Rehab to Home facility with an emphasis on providing outstanding customer service and care. Responsibilities include:
- working with Liasions for placement of hospitalized, skilled, rehab to home patients
- verifying and working with clinical and financial team to ensure we are the right place for the referral and can meet their clinical and financial needs
- responsible for ensuring room readiness prior to admit, making sure equipment in place for a smooth admission process
- completing Admission Paperwork and obtaining all documentation needed prior to and upon admission
