JOB TITLE: Admissions Intake Coordinator

REPORTS TO: Program Director

DESCRIPTION: The Admissions Intake Coordinator is responsible for organizing and coordinating all aspects of the intake of new patients seeking care at CHD Venture, LLC. The Coordinator will complete an intake interview with patients and initiate the admissions process. The Coordinator works closely with clinical and support staff to ensure a smooth and timely process of information gathering, decision-making, and scheduling of new patients.

DUTIES AND RESPONSIBILITIES:

● Manage the admissions and discharge process for patients including completing and maintaining all intake/discharge documentation.

● Schedule intake appointments, records and verifies patients' information and prepares medical records on new and returning patients.

● Check-in patients, escort patients to and from waiting room, interviews patients and records information into patients' electronic charts.

● Obtains and collects urine specimens for laboratory testing.

● Prepares treatment rooms for admission screening of patients and disinfects treatment rooms, equipment and supplies.

● Acts as back-up receptionist; performs administrative duties and prepares and maintains reports and records.

● Triage / manage questions, contacts, and fax messages.

● File lab results into appropriate binders and distribute lab results to clinical team.

● Attend weekly staff meetings and in-service trainings.

QUALIFICATIONS

Minimum Requirements

● High School Diploma or GED and/or 2 years’ of addiction medicine or behavioral health experience or similar field/environment serving culturally diverse populations.

● Satisfactory criminal background check.

Preferred Requirements

● Bachelor’s Degree in human services, social work, psychology, mental health, addiction medicine or related field.

● Demonstrates compassion and empathy toward patients entering residential care for treatment.

● Demonstrates ability to problem solve and/or de-escalate patients who may exhibit resistance to care, confusion, or intoxication and is able to respond quickly to challenges.

● Ability to perform duties pleasantly, with a professional demeanor while maintaining CHD Venture, LLC’s ethics and values, respect for patients’ culture, beliefs and abilities.

● Strong ability to effectively communicate to a wide variety of audiences from diverse populations.
