Job Description:

Acclaim Physician Group, working in conjunction with Family Health Medical Home of Tarrant County, is currently looking for an Advanced Practice Provider to join our FHC Center. This is a full time position, providing primary care services to patients experiencing homelessness or experiencing high-utilization.


Job Summary:
The Advanced Practice Provider practices under the direct supervision of the supervising physician and is responsible for assisting with all aspects of patient care, including diagnosis, treatments and consultations within the outpatient setting. This position will also educate patients about preventative care and prescribed treatments; may conduct physicals, order tests and serve as a patient's healthcare provider.


Full spectrum (adult & children). Patients also have mental health and substance abuse issues. We are looking for a compassionate provider who is not afraid of a challenge and has experience working with a similar population. This is a great opportunity to work with complex medical, behavioral and social issues, and to provide compassionate, quality care.


Schedule: Monday-Wednesday (36 hours)


Duties and Essential Job Functions:

  • Collaborates with the physician in overseeing and coordinating the multidisciplinary care delivered to all patients, to optimize quality patient care.
  • Reviews patient medical charts.
  • Makes appropriate patient referrals as necessary.
  • Monitors guidelines for the management of care and for ensuring that the services are delivered in compliance with the guidelines established by JPS Health Network and Acclaim Physician Group.
  • Provides care education to patients and families.
  • Provides evidence-based and compassionate care to all patients as assigned to improve patient outcomes and increase patient satisfaction scores.
  • Maintains a collaborative environment with providers, clinical staff, and support staff.
  • Supports academic, educational and research endeavors.
  • Attends Performance Improvement Programs and Peer Review committee meetings; and is an active participant in the process.
  • Understand and fully incorporate themselves into the broader homeless services community through their health care role.
  • Performs other job related duties as assigned.

Qualifications:

Required Education and Experience:
Master’s Degree in Nursing from an accredited school of nursing OR Graduate of an accredited Physician Assistant Program

Required Licensure/Certification/Specialized Training:
Must have current Texas Physician Assistant license or Texas APRN license (Family Nurse Practitioner (FNP) or Adult Nurse Practitioner certification (AGPC-NP)
Basic Life Support (BLS) certification
Advanced Cardiac Life Support (ACLS) certification
Must have reliable source of transportation
