Nuvance Health is seeking an Annual Wellness Navigator/Nurse Practitioner for our newly created Annual Wellness Visit Program which is a part of our Value Based care initiative. This is an exciting opportunity for a startup role to conduct annual wellness visit in our New York based Primary Care Practices in Dutchess and Ulster Counties. The ideal candidate will be flexible and creative to help define the job and help us build the program. Full time and Part time positions available.
This is an employed position with Nuvance Health Medical Practice New York. We offer a comprehensive compensation package, including competitive salary with opportunity to earn productivity incentive, generous benefits, and allowances for CMEs and Dues.
Job Responsibilities:
The NP provides medical care for patients in collaboration with a physician, assisting patients to recover from acute illnesses while minimizing the effects of chronic illnesses. This position also plays a vital role in ensuring system success in our value based care arrangements with a primary focus on quality and value for patients within our value based contract arrangements.
Job Requirements:
·This position requires a minimum formal education of a Master Degree, FNP, State of NY RN and NP license, and Federal DEA Registration and Board Certified Nurse Practitioner.
·Willingness to work variable schedule 8 hour shifts Monday through Friday between 7:30 AM – 6:30 PM
·The role will require travel to accommodate patients at our various primary care locations in Dutchess and Ulster Counties in New York.
·The successful candidate will be adapt at utilizing technology to provide virtual care as needed.
·Previous experience as a NP preferred but new grads are encouraged to apply.
