We are seeking a Behavioral Health Navigator to join our team at our program in Bethlehem, PA.

Earn $24.09 / hour.

Our Behavior Health Navigator Act as an advocate for a caseload of clients by supporting their integrated health care needs, both physical and behavioral. Utilize your own vehicle (company mileage reimbursement) while meeting with your caseload daily in their homes, community, and at appointments. Coordinate care throughout medical and psychiatric hospitalizations, including aftercare and follow-up requirements. Develop service plans with time limited goals to support clients to increase independence. Conduct assessments with clients to determine outcomes and assist clients with coordinating appointments. Offer assistance in medical and behavioral outpatient services. Emphasis on linking clients to community-based resources. Educate clients and their natural supports about mental health and medical diagnoses. Assisting with client's insurance coverage, lab work, and medication compliance. Focus on holistic approach to wellness while acting as a liaison between client's medical and behavioral health treatment providers. The Behavioral Health Navigator is eligible to receive supervision hours towards licensure including LCSW and LPN.

The ideal candidate will possess the following qualifications:

• Masters degree in Social Work, Psychology, Nursing, Counseling, or related field of study
• 1- 3 years direct mental health care experience
• Experience and training in Motivational Interviewing, co-occurring disorders, trauma informed care, and integrated physical health/behavioral health care
