Riis Settlement seeks a part-time bilingual (English/Spanish) Case Manager to provide culturally competent case management services to immigrant communities in Western Queens. The Case Manager will connect immigrant participants to needed resources and services, with a focus on low-cost healthcare for uninsured New Yorkers through the NYC Care program. We are seeking an individual who takes initiative and has a strong commitment to immigrant rights to join our Immigrant Services team and expand our case management services. This is a one-year grant-funded position available from October 1, 2022 through September 30, 2023, with the possibility of extension if funding permits.

Jacob A. Riis Neighborhood Settlement (Riis Settlement) is a community-based non-profit organization that offers comprehensive services to youth, adults, seniors and immigrants in Western Queens. Our mission is to build and strengthen underserved communities in Western Queens and act as a catalyst for change. The resources, opportunities and support we provide help community members reach their greatest potential and achieve social and economic self-sufficiency. The Immigrant Services program at Riis Settlement was founded to meet the needs of a growing immigrant population in Queensbridge Houses and the surrounding Long Island City and Astoria communities. Through ESOL and citizenship classes, immigration legal services, case management, and community outreach and organizing, we support immigrants to become active community members and to be successful in education, work, and civic life.
Primary Job Responsibilities
- Conduct intake interviews to assess participant needs, strengths, and resources.
- Provide information, assistance, and referrals to connect participants with resources including healthcare, public benefits, and emergency relief programs.
- Conduct case work under NYC Care grant: Follow up with individuals contacted through outreach work to connect them to screening appointments with NYC Health + Hospitals to determine eligibility for NYC Care, and meet target of 40 appointments per month.
- Advocate on behalf of participants to ensure receipt of identified services, including scheduling appointments with service providers and following up as needed.
- Maintain participant records and enter information into databases as required by funders, including intake information, progress notes, and any data required for grant reporting.
- Participate in trainings, meetings and campaigns related to NYC Care and other key issues in the immigrant communities we work with.
- Develop relationships with other service providers in the Western Queens community – including CBOs, schools, and elected officials – to conduct outreach and referrals.
- Help train and supervise interns to provide individual case assistance, as needed.
- Assist Immigrant Services Program Director with other tasks and programming
Statement of Skills, Knowledge & Abilities
- Bilingual English/Spanish required.
- Bachelor’s degree in social work, social sciences, or other related field.
- One year of experience providing case management and/or other relevant services to low-income community members.
- Knowledge of public benefits and resources available to immigrants in New York City preferred.
- Must have excellent interpersonal skills, including oral and written communication, and ability to engage with participants from diverse linguistic and cultural backgrounds.
- Detail-oriented and strong organizational skills with ability to work independently.
- Proficiency in Microsoft Office Suite.
