Organizational Description:
Sunnyside Community Services (SCS) is a multi-service, community-based organization providing a broad range of services to approximately 16,000 people of each year. Our services include: pre-kindergarten and after school programs; college prepand career programsfor teens and young adults; supports for immigrants; a vibrant senior center, dedicatedhome care services, support and assistance forfamily caregivers, and social adult day programfor people with cognitive and physical impairments.
Bilingual Case Manager
Job Summary:
Helps impaired older adults to maintain independent life safely in the community
by linking them to the community resources/home services and assisting/advocating to
obtain benefits and entitlements. Works with clients and their families to formulate
appropriate care plans maintaining contact by phone and conducting home visits.
Essential Duties and Responsibilities:
The Case Manager may also perform other duties as assigned
Screens all requests for services, determining clients’ eligibility, and, providing information and referral.
Conducts home assessments to determine: living conditions; functional limitations; need for home care, medical care, socialization, counseling; financial status as relates to eligibility for entitlements/benefits; plan of care/agreement for home services.
Maintains contact with clients, caregivers, and families, staying abreast of any changes in clients’ status or need for further immediate intervention.
Advocates for clients regarding benefits/entitlements/medical care.
Refers clients to emergency community services such as mental health (Mobile Crisis) and/or Adult Protective Services when necessary.
Develops and maintains contact with community/government agencies.
Maintains statistics, including written records of contacts with clients.
Participates in educational trainings and conferences.
Conducts intakes by phone or in person on a weekly basis.
Maintains client and agency confidentiality
Specialized Skills and Competencies:
- Interpersonal and Customer Service Skills: This role involves the Case Manager’s need to foster a positive and working relationship with staff in the program as well as outside agencies. The Case Manger has to have the ability to work as part of a team; to be professional and respond in a timely manner to clients inquiries and concerns.
- Problem Solving Skills: This role requires the ability to have good judgement in solving problems and challenges that arise.
- Time Management skills: Must adhere to and complete their work within the program timelines and guidelines required by DFTA standards.
- Verbal and Written Communication Skills: Must be able to communicate effectively with clients, agencies and staff. Must also have good documentation skills.
PHYSICAL REQUIREMENTS:
Bending, sitting, stooping and standing, walking up and down stairs.
WORKING CONDITIONS:
Office setting. Field visits with potential exposure to client illnesses.
Requirements:
1-MSW/BSW and two years supervised experience in Human Services Field.
2- Bi lingual Spanish/Greek or Polish
3- Driver preferred
