Responsibilities:
- Capture list of potentially suitable home health patients each week through Medalogix platform and (PCP, diagnosis, contact info, HH marketing team, HH referral source, HH clinical team)
- Research each patient to see if they have qualifying diagnosis and reach out to home health clinical team to confirm
- Make first call or visit to patient/family and provide full EOB and if they are interested,hand off to the appropriate hospice marketer
- Communicate each step of the process to appropriate team (home health marketing/clinical and hospice marketing/clinical/intake)
- Establish and maintain positive working relationships with home health clinical team to ensure they send patients in need and the weekly list isn’t the only source
- Weekly check-ins with home health clinical management and care teams
- Help transition hospice patients to home health when they have extended prognosis.
- Help track hospitalized patients to ensure they are discharged with Bridge care in place
- Ride along with home health and hospice marketers to introduce program to facilities
- Performs other duties as agreed on by the Bridge Navigator Supervisor and Chief Sales Officer.
- High School Diploma or G.E.D. required. Bachelor's degree preferred.
- At least three years of experience in health care marketing management and/or home care operations preferred. At least one year experience in sales is preferred
- Minimum of two (2) years of experience in general nursing practice, home health experience preferred. ( If LVN or RN)
- Strong professional presence, a willingness to travel, and strong interpersonal, communication, public speaking and organizational skills.
- Excellent understanding of performance improvement, quality assurance, and utilization management.
- Ability to work well with others, function well under pressure, and work independently.
- Demonstrates excellent observation, verbal and written communication skills.
- Complies and Maintains current CPR certification.
- Participation in Quality Assurance/Performance Improvement Program and agency sponsored training.
- Maintains Driver’s License and Automobile Insurance coverage.
