Job Description

The Reentry Mentor will be a credible messenger (survivor of incarceration or violence) who leads mentorship and case management for individuals transitioning to community life after period(s) of incarceration in city jails.

Reentry Mentors will use wraparound case management and individualized client plans to help individuals, ages 18 and up, navigate life in the community following city jail sentences or pretrial detention. They will work with clients to help them develop positive behaviors and build their emotional, social, and professional development, with the goal of becoming employment-ready and ultimately attaining employment and stability.

Responsibilities include but are not limited to:

  • Coordinate and collaborate with Transition Coordinator to track client progress and needs as they approach release from jail
  • Work with Transition Coordinator to develop relationships with client’s family and potential existing support systems in the community
  • Conduct behavioral assessment and intake for clients post-incarceration
  • Provide case management and mentorship for 20 to 30 clients at any given time
  • Form key part of client’s support system, and guide them in developing strong decision-making, self management and interpersonal skills
  • Work with clients with substance dependence issues to address drugs/alcohol patterns that prevent progress towards stability and employment success
  • Develop long-term community-based support systems for clients
  • Develop and coordinate a tailored service plan for each client
  • Make referrals to community partners to ensure client is fully supported and liaise with referral organizations to monitor and support client progress
  • Follow up with referral agencies to ensure client’s social service, health and mental health needs are being met
  • Monitor and draft notes on activities and client progress, including internships and employment opportunities
  • Use Cognitive Behavioral Therapy and Motivational Interviewing to help clients develop positive thinking patterns and behaviors
  • Facilitate tech-based job readiness curriculum that includes soft skills training, along with training in Microsoft, Excel, PowerPoint, and Microsoft Office
  • Help clients develop resumes, cover letters, and attain proper work documents
  • Deliver relevant curriculum to clients in one-on-one and group settings
  • Participate in regularly scheduled on-going trainings and supervision
  • Develop and maintain community partnerships, including relationships with community-based agencies, other service providers, and community resources
  • Personal development in light of the mission, vision and core values of BronxConnect
  • Promote and advance the work of BronxConnect through professional contacts
  • Monitor attainment of programmatic outcomes
  • Administrative responsibilities, including submitting documentation for funding for events, activities, etc.
  • Write daily, weekly, and monthly notes, as necessary, and submit to Records Administrator, Clinical Supervisor, and Workforce Director
  • Complete various trainings and ongoing professional development opportunities
  • Attend team meetings and case conferences
  • Other related duties as assigned

Qualifications:

· Ideal candidate should have experience working with low-income or system-impacted populations

· Experience administering similar programming for clients, including leading small group activities

· Applicants who are formerly incarcerated or survivors of violence are encouraged to apply

· Excellent observational, listening, assessment, problem-solving, and communication skills; ability to work as a member of a team

· Comfortable working in a South Bronx/Harlem community-based setting

· Excellent spoken and written presentation skills

· Attention to detail and ability to maintain written documentation

· Experience working with Latino & African American individuals or families

· Fluency in Spanish a plus

· Experience in program development preferred

· CASAC certification a plus

Job Type: Full-time

Pay: $20.00 - $23.00 per hour
