KEY RESPONSIBILITIES OF THE ROLE

Care Coordinator will be the entry door of the clinic all patient-related, front-office and back-office tasks. The role will assist in care coordination of the patient experience including the following responsibilities:

  • Coordinate appropriate responses to inbound calls, texts, and emails from patients
  • Schedule appointments for patients
  • Triage communication from various channels
  • Get medical records ready for the consultation with our providers
  • Handle back office operations such as enrolling new patients, documenting patients' EMR, and quality assurance.
  • Conduct health related patient questionnaires as needed.
  • Answer phone calls and electronic messages from pharmacies, labs, DME companies, community providers or referring physicians
  • Document all activities for quality and metrics reporting through the member management application.
  • Assisting with obtaining referrals and authorizations
  • Utilize multiple company database programs for accessing member information
  • Achieve individual performance goals for productivity and quality
  • Other projects as assigned

REQUIRED EXPERIENCE & SKILLS

  • High school graduate or equivalent.
  • Previous customer service, contact center, healthcare experience are preferred.
  • Proficient computer skills and ability to learn new systems.
  • Displays good effective verbal and written communication.
  • Dependable and can effectively prioritize tasks.
  • Exhibits excellent organizational and time management skills.
  • Displays team player qualities.
