Greater Family Health is a multi-site federally qualified health center providing comprehensive medical, dental and behavioral health services within the Chicagoland area. Your Goal:
This position is responsible for facilitating coordination and continuity of care for all Greater Family Health patients.
How You'll Help Patients:
- Facilitates patient referrals to specialty care.
- Assist patients with Prescription Assistance Programs (PAP).
- Assists patients in obtaining prior authorization for diagnostic studies.
- Facilitates coordination of care to achieve individual patient clinical measures.
- Ensures compliance with Greater Family Health quality assurance and risk management standards.
- Performs community outreach and education creating linkages to other health facilities, physicians and social agencies to increase awareness of Greater Family Health’s activities and presence in community.
- Reports to all scheduled work sites and shifts on time and fully prepared to engage in all job responsibilities; location of work sites and shifts will change at the discretion of the supervisor.
- Engages in all offered training, consultations and supervisions made available. Fully and professionally respond to and implement directives and advice offered by all supervisory and training personnel, as we as professionally offered feedback from coworkers.

What You'll Need:
- High School Diploma or GED Certification is required; associate or bachelor degree is preferred.
- Excellent communication skills.
- Computer and office software proficiency.
- Bi-Lingual in Spanish and English is preferred.
