Case Manager/Intake Coordinator

We are currently seeking an Case Manager/Intake Coordinator for a full-time position in our Malibu, California location.
Job Duties and Responsibilities:
- Assist with coordinating client appointments with doctors and psychiatrist.
- Assist clients that might be upset during their break and talk with them if they are having an issue with a therapist.
- Responsible for doing screenings and intakes.
- Complete discharge treatment planning and treatment plan reviews.
- WRAP and utilization reviews.
- Provide case management.
- Linkage and referrals for continuing care.
- Coordination of solid discharge planning and relapse prevention planning.
- Provides case management interventions for relapse prevention and stress management.
- Provide positive community linkages with supportive relationships that promote wellness.
- Act as the liaison for outside caregivers, sober livings and other appropriate community agencies.
- Assist the Office Manager if they need help around the office with client's care.
- Responsible for answering phones.
- Learn about the programs offered at Serenity to be able to provide information to the incoming calls.
