About the Job

Case Managers are responsible for providing young adult to geriatric individuals with person centered, strength-based, time-limited therapeutic supports and clinical interventions that focus on each individual’s recovery. Provide assessment of needs, intensive clinical interventions and recovery tools, in a therapeutic setting or mobile outreach that promotes stability.

This role is part of a brand new program (Fall River PACT) which will provide intensive treatment, rehabilitation, and supportive services for individuals experiencing multiple life challenges, such as homelessness, unemployment, addictive disorders, legal involvement, and/or significant medical conditions.

Qualifications

· Bachelor’s degree in Human Services field, such as sociology, social work, psychology, political science, history, criminal justice, theology, counseling, or education is required

· Portuguese speaking preferred
