Unity House is now hiring a Case Manager for the Transitional Apartment Services Program (TAS). The right individual should have experience, compassion and understanding of working with people living with mental illness.

Responsibilities for the position include but are not limited to:

• Provide supportive services to program consumers with mental illness in a manner that is in keeping with agency mission and service philosophy
• Maintaining charts and documents within the guidelines of Office of Mental Health
• Assist with daily living skills including light to moderate cleaning as needed
• Communicating with providers on a regular basis.

Please make sure you have:

• High School Diploma or equivalent and one-year qualifying experience
• Experience working with multiple populations preferred.
• Must have reliable transportation
• Valid NYS driver's license and acceptable driving record as determined by our company's liability insurance policy.
