Job Summary:
The Case Manager is responsible for the advocacy, care coordination, case record maintenance, and communication with insurance companies of individuals with acquired brain injuries. Duties include:
- Collects information about service recipients and their support systems using interviews, case history, and observation.
- Confers with Clinical Director and treatment team, individually and in team meetings, to exchange, discuss, and evaluate the information for planning, modifying, and coordinating treatment programs.
- Reviews service plans and performs follow-ups to determine service quantity, quality, and satisfaction.
- Prepare service plans, progress summaries, meeting minutes, progress notes, and other documentation as needed.
- Presents written and verbal information to team members, service recipients, family members, payors, and other stakeholders in an understandable, timely, and helpful manner.
- Obtains insurance authorizations and ensures clients do not exceed the authorized amount by tracking and obtaining follow-up authorizations.
- Regularly meets with clients and/or their guardian(s) / support team to review care, clinic policies, and changes to service plans.
- Participates in process improvement initiatives.
Minimum Qualifications:
- BA/BS in Rehabilitation, Education, Social Work, Psychology
- More than two years of experience in Human Services
- Ability to work 40 hours weekly between 8 am- 6 pm, Monday through Friday. This is a flexible position and has no on-call or weekend obligations.
- Valid driver’s license permitting operation of a vehicle in the State of Maine with a satisfactory driving record
- A Criminal background check that meets Agency standards
Preferred Qualifications:
- Experience in the treatment or case management of Acquired Brain Injuries
- Experience with electronic medical records, Microsoft Office software

Experience
Required- Greater than 2 years experience in human services.
- BA/BS in Rehabilitation, Education, Social Work, Psychology or equivalent work experience
- Criminal background check that meets Agency standards
Education
Preferred- Bachelor's - Social Work or better
- Bachelor's - Psychology or better
- Bachelor's - Education or better
Licenses & Certifications
Required- Drive. Lic. - Class C
- Licensed Social Worker
