JOB REQUIREMENTSEducation
Must be licensed as a Social Worker, or a Masters Social Worker, and be a graduate of a Bachelor of Nursing program or a Bachelors or Masters in Social Work program.
Excellent verbal and written skills to facilitate communication with a variety of medical and technical personnel. Participates in continuing education, i.e., seminars, workshops and educational materials. Computer skills for data collection, comparative analysis, graphic software, word processing and spread sheet usage.
ExperienceCertified Social Worker with a minimum of 2 yearsexperience experience in mental health preferred.PhysicalThis is a safety sensitive position. Please see the St. Bernards Substance Abuse Policy for further information.Frequent sitting, standing, walking, bending, stooping, and reaching. Fine motor skills. Must be able to see with corrective eyewear. Must be able to hear clearly with assistance. Exposure to biological hazards. Frequent exposure to unpleasant odors. -
JOB SUMMARY
Responsible for evaluating, planning, implementing, coordinating options and services to meet the patient’s health needs through communication and available resources to promote quality cost-effective outcomes.
The LCSW will be part of the care management team and responsible for assisting in the management of the continuum of care and work within an integrated treatment setting. Will provide direct care to clinic patients such as diagnostic assessment, individual therapy, family therapy, group therapy, and crisis intervention to patients, as referred by physicians, and work with the treatment team to promote patient health outcomes.
The LCSW team member will partner with the multi-disciplinary team utilizing best practices and relevant data. Primary elements of the social work role include: assessment of the psychological, financial, and discharge planning needs of the patients and families, providing information, support, counseling, and referrals to other levels of care and appropriate resources for a variety of age-specific populations, including neonatal, pediatric, adolescents, adults, and geriatric.
