Job description

As Case Manager, YOU will provide case management services to adolescents and their families in our Colbert County Transitions program, and also serving Franklin County. This is a part-time position, 24-hours per week.

Requirements

• High school diploma or GED accredited by the proper regional board, Bachelor’s degree preferred
• Minimum 21 years of age
• Valid Alabama driver’s license and acceptable driving record with proof of personal vehicle insurance

Job Responsibilities / Essential Functions

• Assist in providing case management services, educational programs, and all required documentation
• Facilitate clinical staff in coordination of support services needed to assist the clients and the client's families
• Complete appropriate assessment tools and establish a case management file and service plan within Program required time limits
• Willing to work with families and clients during non-traditional hours in a variety of settings including office, school, client homes, or other approved community settings
• Assist in counselor/staff consultations on appropriate interventions with clients and assist in adjustments to constructive responses to program services
• Work collaboratively with agencies and maintain a professional working relationship between the Center and other agencies
• Perform basic clerical activities related to daily operations of the program.
• Document activities and incidents as required by procedures
• Participate in staff meetings, training, and in-service meetings as required
• Provide transportation for clients in company or personal vehicle following all procedures and protocols for safety and security
• Have access to personal transportation to meet with client caseload on a regular basis at home, school, and/or job location

Job Type: Part-time

Pay: $10.65 - $11.86 per hour
