Job Summary: The Case Worker uses his/her professional knowledge, skills and experience to address problem issues with clients in an effort to prevent further in involvement with criminal justice or child welfare system. The Case Worker attempts to utilize a multi-system approach, as well as case management skills, to address individual child and family issues. Ultimately the goal is to prevent out of home placement.
Essential Functions and Responsibilities:
Provides casework contact to focus on goal attainment and safety. There is a minimum of two visits per week required; one planned and one unplanned.
Works with the family on assessing needs and the development of the service plan. Educate the family in needed skill areas i.e. budgeting, cleanliness, hygiene, parenting skills etc.
Acts as liaison and facilitator between service providers, the family, and any other agencies i.e. School, mental health, probation, DSS. Minimum contact is every other week or two times per month.
Instrumental in providing timely referrals to outside agencies to assist the family in goal attainment.
Writes family assessment and services plans, responsible for progress notes and maintaining both electronic (CONNECTIONS) and physical files to include internal forms such as case opening, family demographic, client referral, and court reports.
Will attend all scheduled meetings and/or arrange coverage for such i.e. Court, family case conference, family case reviews, DSS reviews, all school meetings, staff meetings etc.
Completes progress notes in a timely manner, within 72 hours of service provision.
Schedule and lead Service Plans, prepares the SP document.
Insure that job-related activities will be in compliance with Glove House’ policies and procedures, OCFS/OMH, relevant professional association ethical standards and accreditation standards.
Is required to safely drive on agency business, as required by job demands. Maintain accurate mileage documentation and submits in required agency timeframes.
Is required to complete and maintain all mandated training within required agency timeframes and within pre-established passing grade levels.
Must maintain timely compliance with all established Agency standards and provide support documents as necessary (physical/TB, driver license, auto insurance).

Collateral Functions:
Contributes to in-service training through presentation and participation.
Maintains information on community resources, which can be utilized during the course of treatment or to make appropriate referral at the time of discharge.
Participates in the process of transferring patients to other levels of treatment as appropriate.
Performs other responsibilities as assigned by the Supervisor.
Minimum Requirements
Bachelor’s degree in Human Services, and two years experience in child welfare, mental hygiene setting or criminal justice.
Must have valid driver’s license and driving record in accordance with agency standards.
Language skills: Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedural manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of organization.
