Job Purpose: It is the responsibility of the CDPAP Specialist to carry out any functions under Best Choice CDPAP.

EDUCATION, EXPERIENCE AND SKILLS:

  • High School diploma. Associates degree preferred.
  • Minimum of one (1) year administrative experience in home care or health care environment.
  • Exceptional computer and technical skills - Proficient in Microsoft office (Word, Excel, PowerPoint, Outlook)
  • Exceptional customer service skills.

SERVICE STANDARDS

  • Develops and maintains positive “customer” relationships (“customer” is defined as co-workers, participants, visitors, vendors, supervisors).

  • Initiates, if appropriate, incident reporting process, and participates in incident investigation and reporting as applicable.

JOB RESPONSIBILITIES
1. Communicates scheduling conflicts or overlapping shifts with the consumer.
2. Completes the consumer’s schedules and makes scheduling changes as requested by the consumer.
3. Conducts announced and unannounced visits to consumers’ residences quarterly to ensure validity and satisfaction of services provided.
4. Conducts internal audits to identify potential errors and reports any discrepancies for resolution.
5. Contacts personal assistants for corrections required on timesheets.
6. Coordinates Personal Assistant registration process.
7. Develops outreach methods, in conjunction with Community Outreach and Education Specialist, to increase consumer enrollment.
8. Intakes CDPAP consumers through field/in office visits.
9. Maintains documents, records, statistics, timesheets and other related reports in an organized, timely and accurate manner as per policy and procedure.
10. Maintains spreadsheets/calendar with daily program activity.
11. Meets monthly census productivity goals.
12. Monitors Personal Assistant compliance and maintains documentation of injuries/accidents at the consumer’s residence.
13. Notifies appropriate personnel of potentially fraudulent activity (Fraud, misuse of services)
14. Participates in weekly CDPAP meetings and reports weekly on program activity.
15. Provides consumer education and guidance.
16. Provides 60-day authorization/PA compliance expiration notification to consumers via mail.
17. Receives initial calls from referral sources and enters intake information into the system.
18. Verifies insurance coverage and conducts monthly eligibility checks in E-Paces to ensure insurance coverage

Physical Requirements: This role will require bending, lifting, sitting, stooping, and standing.
Working Conditions: Most of the work of this role will take place in an office setting and in the field. It may require occasional evening, weekend, and holiday work.

Job Type: Full-time

Pay: $43,000.00 - $50,000.00 per year
