Position Purpose
Rocky Mountain Human Services is hiring active Service Coordinator positions. This role provides coordination to customers through planning, coordinating, and monitoring services provided to Children with developmental disabilities and/or significant medical needs and their families.
Children may receive support through a variety of waivers and state funding including Children's Extensive Support, Children with a Life Limiting Illness, Children’s Habilitation Residential Program, Children s Home and Community Based Services, and the Family Support Services Program.
Essential Duties
- Maintains timely and accurate data, contact notes, and documentation in case client records.
- Facilitates, writes, and monitors annual assessments and service plans for Children with disabilities and/or significant medical needs.
- Monitors client status, utilization, and satisfaction with services and adjusts the service plan as needed.
- Ensures prior authorization for services and enters information timely into the state operated system(s).
- Schedules and conducts quarterly, bi-yearly, and/or yearly family contacts to review support needs with families. Some contacts will include face to face visits in the community at their homes.
- Completes Developmental Disability Determinations for continued program eligibility when applicable
- Assists Children and Families with accessing Mill Levy funding as applicable.
- Initiates and writes referrals for waiver services as applicable.
- Educates customers/families supported regarding various state plan benefits, programs, options, and services.
- May assist in transfers and placements to new waiver programs when needed.
- Establishes professional and effective collaboration, communication, and coordination among all responsible parties of the member’s interdisciplinary team.
- May facilitate interdisciplinary team (IDT) meetings as requested or as needed.
- Advocates for the best interest of the child in services.
- Maintains professional and ethical manner with all interactions and meets performance, quality, customer service, and coordination standards as assigned by the department management team.
- Participates in training and staff development opportunities.
- Actively participates in team meetings and communicates progress and barriers with Supervisor and/or Program Manager or Department Director.
- Performs other duties as assigned.

- Works with clients, families, and appropriate community agencies to coordinate and facilitate services for clients.
- Ability to meet/work with clients and their families in a variety of settings.
- Maintains family/caregiver contact.
- Attends staff, team, and department meetings.
- Attends in-services, staffing, and other meetings with supervisor’s approval. May be appointed to committees.
- Access to reliable transportation to complete work within established required timelines
Knowledge, Skills and Abilities
Ability to receive, process, and maintain information through oral and written communication effectively Knowledge and understanding of Children with developmental disabilities and/or significant medical needs- Knowledge and understanding of State Funded and Medicaid Waiver programs.
- Ability to maintain strict confidentiality

Required Qualifications
Bachelor’s degree in Human Services (or related field), or five years working with people with developmental disabilities and their families, or a combination of education and experience.
Preferred Qualifications
- Fluent in Spanish
- Prior case management experience
