Responsibilities:
POSITION SUMMARY: TH Clinical pharmacist works directly with physicians, other health professionals, and patients to ensure that the medications prescribed for patients provide the best possible health outcomes.

EDUCATION
: Graduate of an ACPE-accredited school of Pharmacy

EXPERIENCE
: Hospital experience or pharmacy residency

LICENSURE/CERTIFICATION
: Licensed as a Registered Pharmacist in the state of South Carolina and completes continuing education credit hours as required by the South Carolina Board

ESSENTIAL FUNCTIONS:
1 – Order entry review/verification. The clinical pharmacist provides prompt and accurate reviewing of physician orders including but not limited to correct dose and frequency. (25)

2 – Review of assigned patients. The clinical pharmacist develops, monitors and/or recommends a plan for optimizing the patient’s medication therapy in collaboration with the physician(s) on the patient case. (25)

3- The clinical pharmacist utilizes TH safety surveillance system. (25)

4 – Attendance of SIBR and pulmonary rounds. The clinical pharmacist attends and actively participates in interdisciplinary rounds. (10)

5 – Review of policies and protocols. The clinical pharmacist reviews and updates assigned policies and protocols to be in line with current regulations and evidenced based guidelines. (15)

KNOWLEDGE/SKILLS/ABILITIES
:
Pharmacist must demonstrate and apply knowledge of pharmacology, pharmacotherapy, pathophysiology, and the clinical signs, symptoms, and natural history of diseases and/or disorders to assist in providing an appropriate pharmaceutical plan for the patient.

Communication, supervisory, organizational, learning and teaching skills essential to the proper performance of the duties assigned.

Pharmacist must have ability to:
Work under direct or minimal supervision
Work cooperatively with health-system and pharmacy staff
Handle frequent interruptions and adapt to changes in workload and schedule
Multi-task and problem-solve
Set priorities, make decisions, and respond quickly to emergency requests
Provide excellent customer service
Handle stress appropriately
Provide effective, cost-conscious care
Provide drug information to hospital employees and medical staff
Adhere to state and federal laws governing the practice of pharmacy
Adhere to institutional protocols regarding medication use
Report medication errors and adverse events
Perform and document clinical interventions
Oversees preparation and dispensing of medication for inpatient use including sterile compounded products
