Company Overview:
Communities for People (CFP) is a non-profit agency serving youth and families through community-based programs and services. We are looking for a Clinician for a residential Program. You will be instrumental in helping youth and families to achieve permanency, independence and integration into the community. Job Summary:
You will be responsible for coordinating and developing appropriate treatment plans for youth and families referred to our Emergency Shelter STARR program (Stabilization, Assessment. Rapid Re-integration).
STARR Clinician Responsibilities:
- Coordinate entire intake process for clients
- Develop and regularly monitor individual treatment plans for each client including required assessments and treatment reviews
- Provide clinical input to team regarding client's treatment
- Provide crisis intervention and conduct meetings with clients in crisis
- Attend all treatment meetings and crisis meetings for clients
- Conduct regular case and discharge conferences on all clients
- Coordinate all dental, medical, psychiatric. psychological, and therapy services for clients as needed
- Provide vocational guidance and arrange home visits and family contacts for clients
- Ensure files are up to date and progress reports are written and forwarded to appropriate stakeholders as well as kept up to licensing/accreditation standards
- Review and submit progress notes to referring agency monthly
- Work with other community resources, services, courts, state agencies and schools, and act as liaison to state agencies licensing staff
- Attend all weekly meetings, in-service trainings, and orientation programs
- Be on-call to staff and provide emergency staffing when necessary
STARR Clinician Requirements:
- Master's in Social Work or related field
- Knowledge of community resources and service planning
- Excellent writing skills and time management
- Valid driver's license and vehicle
- 1-3 years clinical experience in non-profit environment; preferably experience working directly with youth and families
