The Tribal Health Department is searching for a full-time, benefited, Community Health Representative to join their team!
This position serves as a Tribal liaison and advocate for Tribal citizens regarding safety, health and wellness. The work involves regular interaction with the Tribal community and local health care agencies and organizations, accurate compilation of data and careful record keeping. Responsibilities include health screening, providing health education and disease prevention information to Tribal members, planning and coordinating health education activities and working cooperatively with Health Department and Social and Community Services Department staff to plan and conduct activities that promote healthy lifestyles among Tribal members. The CHR plays an important role in connecting Tribal citizens to all other Tribal programs and services.
Essential Functions
- Provide basic direct health services and health education to the Tribal community, either in their homes, tribal office, or during Tribal community gatherings
- Under the direction of Medical Clinic staff, perform diabetes, cholesterol, BMI and other health screening of Tribal members. Monitor blood pressure, weight, and other indicators of health
- Keep patient records up to date and maintain confidentiality of records
- Serves as a liaison and advocate for Tribal citizens and families. Assists Tribal citizens and families to follow care plans provided by Jamestown primary care providers. Assists Tribal citizens in applying for and accessing services from social and health service agencies that are designed to help meet the health and social needs of clients
- In coordination with the Jamestown Medical Provider teams, act as a case manager by assisting Tribal citizens to coordinate health services from various health care providers and agencies
- In limited situations, provide transportation to Tribal citizens to assist them to keep appointments for health-related services and functions
- Develop health related newsletter articles for the monthly Tribal and HHS newsletter
- Assist the Tribal Health Benefits Manager in the development, planning and facilitation of health and wellness related events and programs for the Tribal community. Attend Wellness Programs and assists community members as directed by supervisor
- Attend workshops to remain educated in issues and information related to the health profession
- Maintain current certification associated with job duties
- Adhere to all JST policies and procedures, including but not limited to standards for safety, and work practices as described in the Occupational Exposure Control Plan and HIPPA
- Perform additional duties as assigned.
Requirements
- High School Diploma or equivalent required
- Previous experience providing basic health assessment and health related care is desirable
- Ability to understand and relate to the unique health needs of a Native American Community
- Knowledge of the cultural, social and economic characteristics of American Indian people
- Must possess demonstrated written and verbal communication skills
- Ability to work independently, meet deadlines, and prioritize workload
- Knowledge of basic nutrition and health guidelines, medical application procedures and ability to model and promote Wellness
- Must obtain Health Assistant Registration, First Aid and CPR certification within six months of employment
- Computer proficiency in Microsoft Suites
- Exceptional interpersonal skills including effective verbal and written communication
- Ability to travel locally and regionally as assigned
- Valid Washington State Driver’s License
- Must be able to successfully pass criminal background investigation
Preferred Qualifications
- State Certified Medical Assistant preferred; or high school graduate or equivalent and Community Health Worker and Caregiver Training required
- Knowledge of Local, State, and Federal Laws and Regulations
- Knowledge of office administrative operations, including personnel management, and compliance
- Excellent organization skills and the ability to prioritize multiple tasks
- Effective time management, decision-making, and problem solving skills
- Demonstrate initiative, work independently with little supervision
- American Indian/Alaska Native preferences apply

Pay: $16.63 - $21.62 per hour
