BASIC JOB FUNCTION:

IMAN’s Community Health Workers (CHWs) will conduct outreach, provide referrals, coordinate community health events, and provide benefits navigation for residents in Chicago Lawn and Greater Englewood. CHWs will work closely with the IMAN Health Center team to provide a range of integrated supports and services through a peer education, trauma-informed, and relationship building lens to impact quality of life and health outcomes.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

● Connect residents in Chicago Lawn and Greater Englewood to peer support, referrals, and resources to improve access to health and wellness resources by using strategies including canvassing, partnerships with local institutions (schools, places of worship, local businesses, etc.), participating in community events and meetings, relationship building with local residents, etc.

● Provide financial counseling services, including, but not limited to medical application assistance, and conduct fee assessments and sliding fee scale determinations as appropriate and necessary.

● Increase access to health education and literacy by coordinating events and workshops for community residents

● Compile and maintain up to date resource and referral lists for use across the IMAN Health Center

● Participate in orientation of new patients to IMAN Health Center; ensure that the patient receives materials and information pertinent to IMAN Health Center policies on sliding fee scale, hours of operation, etc.

● Schedule appointments for both new and established patients; enter necessary patient/guarantor data information accurately and efficiently into the electronic health record; document and verify patient insurance

● Other duties as assigned

EXPERIENCE AND QUALIFICATIONS:

● Minimum 2 years experience working in a community-based setting

● Primary residence in Chicago Lawn or Greater Englewood

● Comfortable with computers, ability to learn to navigate the electronic health record

● Familiarity with Microsoft Office suite preferred

● Bilingual in Spanish or Arabic strongly preferred

● Working knowledge of health care plans a plus

● Commitment to improving community health

● Lived experience related to prevention, management of health conditions

EDUCATIONAL REQUIREMENTS:

● High school graduation or GED

PHYSICAL REQUIREMENTS AND WORKING ENVIRONMENT:

● Ability to sit or stand for extended periods of time

● Willingness to be in the field 3-4 days per week

● Ability to work for extended hours with a computer

● Ability to work flexible hours

● Ability to lift to 50 lbs.

● Ability to work in extreme temperatures

Job Type: Full-time

Pay: $45,000.00 - $50,000.00 per year

• 8 hour shift
• Monday to Friday
• Weekend availability
