Title: Community Nurse

Reports to: Selectboard

Overview: The focus of the community nurse is on home visiting, teaching, coaching, and integration of services as well as prevention of disease and promotion of health for individuals and the community.

The community nurse is an independent position. S/he works with the primary care provider, town officials and other involved organizations as needed. Clients are not charged for the care provided by the community nurse. This position does not take the place of visiting nursing services covered by insurance.

Responsibilities include

• Making home visit to assess physical and social well being
• Developing care plans
• Documenting interventions and outcomes in REDCap system
• Assisting with advanced care planning
• Making referrals to other organizations including area on aging, food banks, and social service agencies
• Assessing the needs of the community
• Educating the community about health, nutrition, hygiene, and disease prevention
• Attending Community Nurse Connection monthly support meetings via zoom

Qualifications:

· Bachelors in Nursing preferred, active VT RN license

· Strong health assessment skills

· 5 plus years of experience in with home visiting such as VNA or hospice

· Understanding of local resources

· Maturity in the nursing profession

· Excellent time management skills

Job Types: Travel nursing, Part-time

Pay: Up to $10,000.00 per year

Standard shift:

• Day shift
• Evening shift

Supplemental schedule:

• On call

Weekly schedule:

• Monday to Friday
• Self-determined schedule
• Weekend availability

Experience:

• Nursing: 1 year (Preferred)

License/Certification:

• BLS Certification (Preferred)
• RN (Preferred)

Work Location: On the road
