Community Resource Navigators are an essential component of the patient-centered, integrated health care model. Community Resource Navigators work with patients to assist in addressing non-clinical barriers to their care. These navigators focus on connecting patients to community resources that may be needed to support the overall care of the patient. This position works in collaboration with providers and clinical staff to address these non-clinical needs of the patient.

PRIMARY ACCOUNTABILITIES

  • Interacts and engages patients regarding resource needs (i.e.) food bank, housing, energy assistance, homeless services, etc.
  • Assists in arranging transportation services and/or interpretation services when needed
  • Facilitates and assists with connecting patients to financial support resources such as, Patient Assistance Programs
  • Assists patients with completing forms/paperwork if needed
  • Assists patients in signing up for health insurance through the Washington Health Exchange
  • Connects patients to programs and services such as community education services, parental support, employment training, etc.
  • Assists in bridging patients with others both in the community and internally for resources they may need which the Navigator cannot provide directly.
  • Works with patients by patient self-referral or by request of provider team after identifying an acute or chronic need
  • Keeps informed about changes in community resources.
  • Collaborates with Community Liaison about new resources and developing community partnerships with NOHN.
  • Other duties as assigned

POSITION REQUIREMENTS

Education and Qualifications

  • High School graduate required; AA, BA or BS preferred
  • Previous medical setting experience with patient interaction preferred but not required
  • Completion of training in the Washington Health Insurance Exchange
