Summary:
The Community Support Specialist provides direct service to members through outreach (60% of your workday will be spent out of the office providing direct service), including intensive case management, advocacy, linkage and referral, crisis intervention, and collaboration with providers. The community support specialist
Responsibilities:
- Perform weekly/monthly wellness checks after client transition.
- Assist in addressing the needs/objectives of the clients relative to their supported service within the community
- Assist client in establishing a housing expense budget
- Assist client in move-in plan and furnishing of the unit plan.
- Registering Client in PAIRS system.
- Assist client in purchasing furnishing for permanent supported housing.
- Maintain all receipts of transitional funds to provide to subsidy Administration
- Assist client in setting up utilities for permanent supported housing.
- Ensure accurate recordkeeping of all client income and all sources at entry, minimally at each annual recertification, and at the exit to determine income increases.
- Help clients identify and build upon their strengths.
- Assist clients in gaining information and support from the community to make their goals a reality.
- Support clients in problem-solving and decision-making.
- Model healthy interpersonal skills.
- Advocate for services needed for clients served.
- Keep client case files up to date and organized.
- Communicate regularly and effectively with the client, service coordinators, service providers, and support personnel to ensure that clients meet house application requirements and maintain housing when secured.
Minimum Qualifications:
- Associates or Bachelor’s degree with five years of experience in the Human Services Field working with individuals with mental health issues
- Flexibility to work evenings and occasional weekends
- Demonstrated ability to organize and manage multiple projects and cultivate relationships
- Expertise in providing client support/excellent customer service
- Must have reliable transportation
- Computer literacy and efficiency with MS Office software
- Excellent verbal and written communication skills
- Must commit to working from a strengths-based perspective.
