POSITION SUMMARY

The Community Support Worker is responsible for supporting Prestige Healthcare Resources Inc. Consumers to obtain resources and skills needed to function at optimal capacity and life satisfaction while living with a mental disorder. The CSW works closely with therapists and psychiatrists to coordinate care to meet client goals. Additionally, CSWs coordinate care for consumers’ health needs, housing needs, vocational needs, entitlements and other domains as individualized to the consumer.

DUTIES AND RESPONSIBILITIES


  • Conduct Intake with participant consisting of Dual Diagnosis Assessment, Crisis Plans, Demographic information and reason for seeking services
  • Create Treatment Plans under the supervision of a master’s level professional for participants receiving only Community Support Services
  • With consumer, identify needed resources and assist consumer to obtain resources
  • Provide individual interventions for the development of interpersonal and community coping skills, including adapting to home, school and work environments
  • Assist and support clients in crisis situations
  • Outreach clients to enhance engagement in treatment and meet “client where they are at”
  • Provide services in natural settings to include home, school, court, other social service agencies.
  • Provide referrals to outside agencies
  • Educate, support and consult to client’s families and their support system, which is directed exclusively to the well-being and benefit of the client
  • Assist the client in symptom self-monitoring and self-management for the identification and minimization of the negative effects of psychiatric symptoms which interfere with the client’s daily living, financial management, personal development, or school or work performance
  • Notify clinical manager when client is non-compliant with service goals
  • Participate in case reviews with other team members
  • Document all contacts, referrals and other necessary information
  • Maintain participants records that are accurate, comprehensive and in which the documentation is completed on a timely basis
  • Understand and apply state and federal confidentiality regulations about participants’ records and correspondence
  • Perform other duties as assigned by the Program Director
  • Maintain monthly statistics and provide reports as needed to Program Director

QUALIFICATIONS

Education and Experience Required

  • Requires a bachelor’s Degree in Social Work, Counseling, Human Services or related field; or
  • 3 years of relevant work experience with 2 of those years providing mental health community support
  • Must be fluent in Spanish and/or English.
  • Valid Driver’s License needed as well as access to automobile.
  • Working knowledge of Windows and Word
  • knowledge of patient confidentiality issues and HIPAA compliance/regulations are essential.
  • Must have at least 3 years of caring for ACT consumers.
