General Summary: Assist the team leader with the overall day-to-day activities of the teams by providing leadership and clinical support to other staff on the treatment team.

Salary starting at $27 an hour

Responsibilities:

  • Under the direction of the Program Manager, provide group and individual clinical/administrative supervision and leadership to assigned team members in the implementation and coordination of Health Home activities.
  • Under supervision of the Program Manager, ensure staff is trained in the development and implementation of client treatment plans that adhere to medical necessity guidelines and that incorporate health home services (Healthcare coordination, wellness and prevention, transitional care, individual and family support services, and comprehensive care management)
  • Monitor the day-to-day activities of assigned team members; Work with Program Manager and Clinical Supervisors to ensure individual and team productivity targets are being maintained (as established by Organization as well as within Health Home billing requirements
  • Provide orientation to new team members; Provide ongoing trainings in concert with Program Manager, Clinical Supervisors and Lead RN to educate staff in prevention and health promotion activities, management of health conditions, and coordination with healthcare facilities and other community partners
  • Monitor clinical documentation of assigned team members to ensure treatment plans, progress notes, and other client documents are in accordance with Organization standards for quality, and submitted within established timeframes
  • Provide direct clinical care in support of staff development and client needs; Assist in community networking and education.
  • Under the direction and License of Program Manager, provide direct clinical supervision in accordance with BHDDH Rules and Regulations
  • Identify individual training needs for team members; Monitor performance and staff growth or need for re-mediation.
  • Create and promote effective working relationships among team and colleagues; Work collaboratively as part of a department/team to facilitate the program’s ability to meet its goals and objectives.
  • Maintain and facilitate a positive flow of communication within team, CSP program and The Organization as a whole.
  • Assist Program Manager in monitoring staff adherence to CSP and Organization policies & procedures;
  • Be well versed in clinical care standards to ensure high quality service to clients.
  • Embrace trauma informed principles, guiding relationships with both staff and clients
  • Serve as liaison to the state behavioral healthcare programs to insure facility licensing compliance to state contracts.
  • Assist Program Manager in recruiting and hiring of new staff; Maintain responsibility for completing performance evaluations, including routine monitoring of staff performance and performance improvement plans, as indicated

Requirements:

  • Independent Clinical License (LICSW, LMHC, RN) preferred
  • Master’s degree in counseling or social work or Bachelor’s degree in Nursing required
  • 3 years’ experience working with the Severe Persistent Mental Illness and Dual Diagnosis population
  • 2 years’ supervisory experience
  • Experience working with clients in a community based setting
  • Experience working with a multi-disciplinary team
  • Experience working with the Assertive Community Treatment model
