We are a company that assists individuals in need of support within a group home setting as well as within the community. We are currently seeking to fill multiple mental health associate positions located in Fond Du Lac, Menasha WI, and Oshkosh WI. Mental Health Associates will provide assistance to individuals with developmental disabilities and mental illnesses in their daily cares, meal preparation, household chores, activities, and following each care plan as set forth. The job responsibilities for this position include, but are not limited to:

· Assisting in maintaining medical records, financial records, daily charting, and maintenance aspects of a group home.

· Adherence of individual support and behavioral plans. Policies and procedures, and assessing the needs of an individual on a continual basis.

· Continual communication between co-workers and management

· Having a working knowledge of individuals’ current medical conditions and prescriptions. Attending medical appointments and reporting information back to management

The minimum requirements for this position are; However, we will train the right person for this position.

· A minimum of 1 year of experience working with individuals with disabilities or mental illnesses.

· Medication Administration, Fire Safety, Standard Precautions, First Aid & Choking training

· Good decision-making, time management, problem solving and communication skills.

· Current driver’s license and auto liability insurance.

· Acceptable driving record/criminal record.

· Reliable transportation.
