We are looking for a Director of Admissions to join the team at one of our Skilled Nursing Facilities in Brooklyn, NY!

We have six rehabilitation centers across Brooklyn and Manhattan, NY, specializing in post-acute and sub-acute skilled nursing and rehabilitation. As a Director of Admissions, you would be part of a winning team doing meaningful work to improve residents' quality of life, health, and comfort.

Director of Admissions Responsibilities

  • Oversee the admissions, discharges, and transfers of residents to ensure effective and efficient utilization of the facility’s beds
  • Plan, organize, coordinate, and supervise office personnel activities to achieve the highest level of effectiveness
  • Develop and maintain contacts with discharge planners in the hospitals for potential admissions
  • Convey the residents’ information to the nursing and medical departments
  • Act as a liaison with resident families and facility staff upon admission to the facility to ensure a smooth transition from the community or hospital setting
  • Implement and supervise admitting procedures as established by the Administrator and Department of Health regulations
  • Ensure the accurate completion of all forms relating to the pre-admission and admission procedures
  • Prepare and submit monthly reports to Admission Committee
  • Address prior approvals as needed to submit to the proper drug plans for approval or follow-up
  • Perform all duties that are an essential function of the position

Director of Admissions Requirements

  • High school diploma or its equivalent
  • COVID-19 Vaccination
  • Basic knowledge of insurance and Electronic Health Record (EHR)
  • Must possess the ability and willingness to work harmoniously in a team setting, while also being a self-motivated person

Preferred

  • Previous experience in a Skilled Nursing Facility or Senior Living

Job Type: Full-time

Pay: $80,000.00 - $120,000.00 per year
