The Director of Nursing / Administrator is a key leadership role within Valeo Home Health & Hospice.

ROLE RESPONSIBILITIES

  • Administration of day-to-day clinical operations and quality of care
  • Oversight for the overall quality of the program for Home Health and Hospice Services
  • Responsible for implementing and maintaining administrative practices, agency philosophy, goals, policies, and procedures that align with corporate objectives and ensure compliance with local, state and federal regulations
  • Coordination of care among clinical disciplines and branch administrative team members to ensure patient/family needs are met and exceeded
  • Demonstrates knowledge of key metrics to track and trend operational processes related to home health and hospice services ensuring the branch is on target with identified goals and KPI’s
  • Oversight and responsibility to manage clinical customer complaints, infection control, incident reporting and development of corrective actions plans to facilitate improvement at branch level
  • Oversight of the delivery of an effective clinical on boarding, monthly training sessions and annual training requirements
  • Oversight of clinical case conferences/IDT’s ensuring the highest quality of care with the most cost-effective approach and effective utilization maximizing agency resources
  • Maintains personal knowledge and remains up-to-date regarding home health, hospice state and federal regulations
  • Ensures that the agency adheres to standards of practice acceptable to all care delivery
  • Assist in policy review and revisions as requested to establish and support the highest possible quality of patient care, cost controls, quality assurance and staff performance
  • Ensures established corporate policies are enforced and adhered to at the branch level
  • Oversight of branch quarterly and annual agency strategic planning sessions/QAPI meetings in coordination with corporate leadership
  • Champion for integrating/documenting clinical quality care with utilization of EMR
  • Available to be on-site during normal business hours, or available by telephone when offsite conducting agency business.
  • Available after hours on an as needed basis with on-call responsibilities

EDUCATION AND QUALIFICATION

  • Active, unrestricted Utah RN license, required
  • Current CPR certification
  • Driver’s license, vehicle insurance and dependable vehicle
  • Registered Nurse experience of at least 5 years, 3 years in post-acute setting including home health and/or hospice
  • Minimum 1 year of demonstrated leadership experience
  • Demonstrated level of comfort with use of computer software programs and EMR charting/medical records
  • Comfort level in running clinical data reports, analyzing data, recognizing trends and developing process improvement initiatives through a continuous cycle of improvement.
  • Ability to compile data and prepare in presentation format for quarterly and annual meetings
  • Ability to lead, collaborate, and has a solution focused mind-set.
  • Must be approachable and honest.

Job Type: Full-time

Pay: $92,500.00 - $110,000.00 per year

Experience:

  • Supervising experience: 5 years (Preferred)
  • ADON in Home Health and or Hospice: 2 years (Preferred)
  • Home Health: 5 years (Preferred)
  • Hospice care: 3 years (Preferred)
  • Palliative Care: 3 years (Preferred)

License/Certification:

  • Utah RN license (Preferred)
