The Grand Pavilion for Rehabilitation and Nursing at Rockville Center is looking for a talented and hard-working Direct of of Social Services to join their team.

The Grand Pavilion for Rehabilitation and Nursing at Rockville Center is a facility with 158 beds and is proud of their state-of-the-art Rehabilitation Gym servicing 60 + short term beds. The Grand Pavilion for Rehabilitation and Nursing at Rockville Center team spanning 20 years of experience with backgrounds in acute, sub-acute, long term settings provide an excellent setting for learning and growth.

The primary purpose of this position is to plan. Organize, develop and direct the overall operation of our Social Services Department in accordance with current federal, state, and local standards, guidelines and regulations that govern our facility, and as directed by the Administrator or ensure that the emotionally related emotional and social needs of the residents are met/maintained on an individual basis.

Qualifications:

• Must have a bachelors in Social Work or related field- CSW , LMSW preferred.
• Must have the ability to work independently and multitask.
• Must have a positive attitude and be able to provide excellent customer service.
• Must be familiar with the Electronic Medical Records programs and or computer literate.
• Must possess the ability to work harmoniously with other department personnel.

Functions Include:

• Meet with patient/residents and their family’s to gather information and design a safe, secure discharge plan for each resident..
• Complete all necessary forms/documents for each patient in a timely fashion as per facility dictates.
• Make community referrals for each discharged patient, asindicated.
• Coordinate Social Services activities with other departments as necessary.
• Delegate authority, responsibility, and oversee accountability of social services team.
• Review resident/family complainants and grievances from personnel, families or residents
