Responsible for the review, response, and coordination of care for patients primarily focusing on referrals entered in electronic referral systems and developing the referral relationships of the company within the community, in accordance with company policies and procedures.

Responsible for the review, response, and coordination of care for patients primarily focusing on referrals entered in electronic referral systems and developing the referral relationships of the company within the community, in accordance with company policies and procedures.
Responsibilities
- Serves as the first line of customer service for external and internal customers (care center relationship).
- Professionally responds through electronic referral platforms within the specified time requirements.
- Performs reverse marketing in coordination with the local BD Leader to reinforce messaging, stay top of mind and present information where it may be of value to our customers.
- Meets assigned key performance indicators (KPIs).
- Receives and documents patient referrals to home health services through electronic referral systems, ensuring compliance with applicable coverage criteria.
- Coordinates with care centers on ability to staff electronic referrals and communicates with care center Intake Coordinators or designee to enter referrals into Homecare Homebase.
- Provides the appropriate information from the eReferral system to the Intake Coordinator to ensure complete referral information.
- Communicates pertinent patient information to the care center team.
- Communicates and follows up with Business Development regarding pending admissions or as directed by Director of Operations.
- Complies with policies, procedures and regulatory mandates including but not limited to abiding to the terms of the Amedisys Compliance Program.
- Performs other duties as assigned.
Qualifications
Required
- High School diploma or GED equivalent.
- Experience working in an environment where timeliness and quality of response were measured and impacted business results.
Preferred
- Home health or Hospice experience.
- Experience customer facing in sales or operations.
