Title: Evidence-Based Program Coordinator – Contract Position
40 hours per week
$21 per hour
Description: The Evidence-Based Programs Coordinator will increase participation and sustainability of health and wellness programs offered in the ten county region of Three Rivers (Butts, Carroll, Coweta, Heard, Lamar, Meriwether, Pike, Spalding, Troup and Upson). The coordinator will receive training in various evidence-based programs and will be required to teach the classes in person and virtually. The programs include but are not limited to Chronic Disease Self-Management Program, Diabetes Self-Management Program, Tai Chi, Matter of Balance, Powerful Tools for Caregivers, Aging Mastery, Dealing with Dementia Behaviors, and others. The coordinator will complete all necessary paperwork for the programs, prepare for each session, and will coordinate where the trainings will be held. The coordinator will recruit participants from senior centers, industries, civic groups, support groups, etc. The coordinator will be responsible for logging all units of the trainings in the statewide database. See below for a more detailed list of duties for the first year. This contract will be in effect until June 30, 2026 with the possibility of renewal.
The following services are to be performed:
1. Initially, receive training to become certified in Tai Chi (tai chi for arthritis 1 & 2, tai chi seated, tai chi for diabetes, tai chi for fall prevention), a Matter of Balance (MOB), and Chronic Disease Self-Management Program (CDSMP) in the first three months.
2. Market and recruit participants throughout the community including but not limited to churches, civic groups, industries, senior centers, chambers of commerce, support groups, libraries, gyms, recreational centers, schools, etc.
3. Teach at least one Tai Chi class, one MOB class, and one CDSMP class in the first 6 months.
4. After the first 6 months, receive training to become certified in Aging Mastery, Diabetes Self-Management Program (DSMP), and Powerful Tools for Caregivers (PTC).
5. Teach at least one Aging Mastery class, one DSMP class, and one PTC class in the next 6 months.
6. Log all classes in the statewide database.
7. Complete all paperwork for each program and submit to the Division of Aging Services as required for each program.
8. For subsequent years, become certified in the remaining EBPs.
9. For subsequent years, teach at a minimum of 3 of each of the above mentioned EBP (Tai Chi, MOB, CDSMP, Aging Mastery, DSMP, and PTC).
