What We Are Looking For

Job Title: Hospice Registered Nurse Case Manager

Schedule: Monday - Friday 8:30am - 4:30pm

Location: Sullivan County NY

What is a RN Case Manager?

The RN Case Manager is essential to the Hospice organization. From admission to the end of our patient’s journeys, the RN Case Manager provides professional nursing care by utilizing all elements of the nursing process. The position is our field team leader in our local community. The RN Case Manager leads a designated team that includes a LPN and Hospice Aide Specialist who are assigned to their own personal territory located throughout Orange and Sullivan Counties. In addition to providing hands on medical care, the RN Case Manager collaborates with the Social Work and Spiritual Care departments to develop a comprehensive plan of care.

What We Need

• Candidates must have a NYS RN License
• Candidates must be a graduate of an Accredited School of Nursing
• Applicants must demonstrate basic knowledge of nursing theory and practice, knowledge of appropriate technology, knowledge of practice standards for area of responsibility, sound clinical skills, sound standards of practice, and positive communication skills.
• Previous hospice/home care experience preferred.
• Reliable transportation and valid driver’s license with an acceptable driving record required.
