Job description

Direct Services is seeking to hire a Georgia licensed RN or LPN to work in the Community Care Services Program in the Americus office. Qualifications include two years of experience as a nurse in geriatric nursing, community health, long term care or chronic diseases of adults. Duties will entail assessing clients in their homes throughout a nine county area, determining a level of care for community-based services for Medicaid recipients, and functioning as a member of an interdisciplinary team. Candidate will be a self-motivated team player with excellent communication and organizational skills. This position does not require direct client care.

· Must be a Georgia Certified RN or LPN

· Must have two years of experience in geriatric nursing, community health, long term care

or chronic diseases in adults.
