Job Summary:
Performs tasks to ensure integrity of the medical record through standardized processes and administration of department policy. Assists in compilation, scanning, indexing and filing of patient information into the electronic medical record (EMR). Maintains the facilities medical records and is responsible for their accuracy, security and confidentiality.

Job Summary:
Performs tasks to ensure integrity of the medical record through standardized processes and administration of department policy. Assists in compilation, scanning, indexing and filing of patient information into the electronic medical record (EMR). Maintains the facilities medical records and is responsible for their accuracy, security and confidentiality.
- Performs collection, processing, scanning, filing, maintenance, storage, retrieval, indexing and distribution of medical records according to facility policies and procedures
- Ensures accuracy and legibility of scanned documents
- Prioritizes scanning of time sensitive documents to ensure availability in the EMR
- Follows established processes for identifying and correcting chart deficiencies and errors.
- Obtains all necessary signatures to complete chart, including contacting physicians’ offices regarding necessary signatures/reports
- Obtains all missing chart contents needed to complete medical record
- Maintains a filing system that meets facility requirements for medical records
- Adheres to established procedures for cross-referencing and indexing medical records
- Maintains necessary index references for facility needs and following established procedures
- Develops and maintains an organized storage system for timely retrieval of individual medical records
- Institutes and maintains a check-out and return system for medical records
- Maintains confidentiality, security, and physical safety of facility medical records
- Arranges for appropriate disposal of medical records per facility policies/procedures, if applicable
- Performs other miscellaneous office/clerical duties as needed
- Participates in HIM Department performance improvement activities
Qualifications:
Required
- At least one year experience in HIM job related duties
- Training or courses in business office activities
- Previous experience with medical terminology or coding
- Good communication skills
- Good computer skills
- Strong ethical and moral character references
- Complete understanding of HIPAA requirements as it relates to this position
Preferred
- 5 years previous medical records experience
- Bachelor’s degree
