One of the largest home health and hospice providers in New Hampshire is searching for a Registered Nurse to join their growing team in the Lakes Region. In this role, you will deliver patient care in a home setting, utilizing the standard nursing best practice, including assessment, planning, intervention, implementation, and evaluation. You will effectively collaborate with patients, caregivers, and other healthcare team members while maintaining the standards of professional nursing. Responsibilities:
- Develop and implement an appropriate plan of care that complies with physician/nurse orders, specific treatment measures, expected duration, and discharge goals in partnership with the patient, healthcare provider, and caregiver.
- Document accurate and ongoing assessments, treatments, and discharge of the patient, including problems, capabilities, limitations, and progress toward goals.
- Engage the patient and caregiver in meaningful education as indicated per the care plan.
- Communicate with all physicians and health practitioners involved in the plan of care.
- Coach, mentor, and precept new nursing employees and students.
Qualifications:
- An ADN is required, and a BSN is preferred.
- 2+ years of acute care nursing experience is required.
- Active RN or PN license required.
Additional info:
- Full-time, permanent position
- Hours: Monday-Friday, 8a-430p.
- Pay $32-50/hr., depending on experience.
