Loving Care In-Home Health Services is accepting applications for a full-time Hospice LPN for our Shawnee office. This position will cover Shawnee, Seminole, Tecumseh, Prague and surrounding areas for hospice patients.

JOB DESCRIPTION:

• Position will be responsible for delivery and coordination of care to assigned hospice patients.
• Assess and treat patients according to plan of care developed individually for each patient, including vital signs, administer medications, and give treatments as ordered.
• Symptom management and end of life care for hospice patients and their families.

JOB REQUIREMENTS:

• Active State of Oklahoma LPN license without restrictions;
• Current CPR certification;
• Current valid Drivers License

ABILITIES:

• Ability to work effectively with designated hospice care team;
• Communication and interaction with patients, family members, facilities and physicians;
• Excellent assessment and organizational skills;
• Ability to work independently and have a good work ethic.
