· Masters level degree in Social Work or a related field OR
· Bachelor’s Degree with at least 2 years of experience in the human service field.
· Designation as a Qualified Mental Health Professional (QMHP-C) according to Medicaid regulations. This requires registration atVirginia Board of Counseling - Qualified Mental Health Professionals
· Able to maintain a flexible schedule based on the needs of assigned clients and families.
· Able to be available to clients/families in crisis situations.
· Able to be available to support clients in an on-call capacity for the program.
· Must have a valid driver’s license and meet insurance qualifications regarding driver record.
Role and Responsibilities
1. Provide clinical services to your clients and families primarily in their home or within the community.
2. Make contact with families within 24 hours of case assignment.
3. Promptly begin the assessment of the family to enable the design of treatment goals and plans.
4. Ensure services are delivered in an ethical manner.
5. Provide clients and families with referral and assistance to appropriate community resources.
6. Teach families about the process of locating and securing needed services.
7. Advocate for the needs of clients when necessary.
8. Maintain awareness of and follow all licensing, DMAS, accreditation and agency policies.
9. Attend and participate in clinical and administrative supervision.
10. Participate at all team meetings and any other agency or departmental meeting as determined by supervisor.
11. Complete all annual and bi-annual mandatory training assigned to you prior to expiration date.
12. Maintain compliance with background check requirements prior to expiration date.
13. Attendance at all training mandated by YFT.
- All duties and responsibilities will support meeting the structured program of care outlined in DBHDS and DSS state standards (DBHDS: 12VAC 35-46-800; DSS: 22 VAC 40-151-720, 22 VAC 40-131-610).
15. Other duties as assigned by the CEO or Supervisors.
