This is a permanent, in-person position with one of our amazing partners at a Home Health and Hospice service provider. This facility has great company culture and builds strong relationships with their employees! This position is for 1-on-1 in-home patient care.

Requirements:
Active LCSW License
Previous Hospice experience preferred
This location has shift options that are based upon number of visits.
Office Location for training:
Westminster, CO
Approximate services areas:
South of I70: Littleton, Parker, Castle Rock, Aurora, and Highlands Ranch areas
Colorado Springs area also available
This position is FT and rate of pay is dependent upon type of visit. Pay is $105 per in-person visit and $75 per tele visit.
LCSW license required
Previous Hospice experience preferred
