Starting rate for a minimum of 2 years experience is $18.85/hr.

ESSENTIAL FUNCTIONS

The Clinical Nurse I (LPN) will be an integral member of the health center care team. Together with providers, nurses, medical assistants, patient service representatives, behavioral health specialists, and outreach staff, the LPN will assist with day to day operations in the health center, participate in care plan implementation and management for assigned populations, ensure continuity of care and assist with care coordination, and serve as a clinical resource for other health center staff. The LPN will work under the direction of the Nursing Services Manager/RN Clinical Coordinator.

JOB DUTIES

• Serve as clinical resource for medical assistants and other health center staff by facilitating day-to-day operations of the health center, answering clinical questions, assisting with patient flow during clinical sessions, and addressing patient care concerns.
• Perform clinical duties as assigned at health center including patient intake, screening, focused assessment, medication administration/reconciliation, immunization administration, patient education and procedure assist. Act as back up for Triage nurse as needed.
• Ensure safe and high quality patient care through participation in quality improvement initiatives. Participate on committees or attend meetings related to these initiatives as assigned.
• Participate in population health management activities such as pre-visit planning, registry management, care transformation projects, and periodic outreach to specific populations for health promotion and disease prevention interventions. Participate on committees or attend meetings related to these interventions as assigned.
• Provide follow-up on individualized care plans, test/lab results, and referrals/authorizations for patients as directed by the clinician or RN.

• Perform nurse visits for injections/vaccinations, blood pressure checks, contraceptive management, and other patient clinical needs in compliance with clinical procedure guidelines, protocols, and standing orders.
