Job description
Providence Care is looking for a passionate and dedicated LPN / Licensed Practical Nurse to join our Home Health team in the Rock Hill, SC and surrounding areas providing nursing services to patients in their private homes or assisted living facilities.
Full time hours, Monday - Friday 8 hour days with limited on-call responsibilities!
Responsibilities:
As a LPN you work side by side with your RN Case Manager to:
- Coordinate care with a team of professionals, including medical clinical director, medical social workers and other passionate caregivers.
- Collaborate with Physician for new orders, to advise of needs and/or changes.
- Positions open in the following counties Rock Hill, Chester, Lancaster counties.
Requirements:
- LPN/LVN License in the active and unencumbered in the state of SC or compact license state.
- 1 year of nursing experience
- Home Health experience preferred but not required
- Passion for helping others
- Driver's license, reliable transportation and auto insurance
