Westminster-Canterbury on Chesapeake Bay is currently hiring an MDS Coordinator for The Hoy Center.

• Full Time 8am to 4pm, Monday-Friday

What will I do?

Our MDS Coordinator conducts and coordinates and completes the nursing sections of the comprehensive assessment of the residents in the nursing unit including the MDS, CAAS and care plans completion and submission process. Ability to receive and interpret initial feedback and validation reports.

• Responsible for completing the nursing sections and coordinating the comprehensive assessment which includes the completion of the MDS (Minimum Data Set, version 3.0 for Nursing Home Resident Assessment and Care Screening) Full Assessment Form, Quarterly MDS Form, Care Assessment Areas (CAAS), of appropriate participating health professionals and care planning that allows the resident to attain or maintain the highest practicable physical, mental, and psychosocial well-being.
• Coordinates and distributes monthly IDT/PPS schedule listing the residents for assessment, indicating the type of assessment to be performed, the date the assessment is to be completed, and the care plan date, addition list as it occurs.
• Responsible for the completing and printing of any required MDS documents including Face Sheet, Basic tracking Form, Reentry Form, and Discharge Tracking Form. Responsible for completion of the Care Plan. Responsible for preparing the MDS for electronic transmission using appropriate guidelines. Responsible for printing and reviewing monthly QI reports.
• Knowledge of the computer process using Monette UltraCare System in accordance with state guidelines and federal 3.0 guidelines and for training MDS users in computer and MDS process. Must provide ongoing training to staff relating to the MDS process.
• Participate in weekly Medicare meetings to include accounting, rehab, and nursing to establish ARD and level of care. Conducts the weekly Interdepartmental Team Meeting as scheduled/Care Plan Meetings.
• Communicates concerns related to MDS process to ADON, DON, and Health Services Administrator.
• Participates and assists in survey process in Health Care by regulatory agencies.
• Participates in and assures appropriate assessment of residents. Communicates resident problems to physicians as necessary. Responsible for updating care plans and assures Health Care staff are implementing and performing care and activities appropriately and recognizing changes in resident’s status.

What do I need for this role?

• Current RN licensure in the state;
• At least two years of clinical and managerial experience in long-term care; two years of experience as MDS Coordinator
• Certification or evidence of continuing education in gerontological nursing and nursing administration in long-term care
• Current CPR certification.
