Arc Herkimer is looking for a Medical Appointment Specialist. The Medical Appointment Specialist will transport and provide quality direct support and care to Residential individuals on medical appointments; as directed, provide quality direct support and care for individuals in their homes. Assist Supports & Services programs as needed. Function as liaison with medical providers, families and staff.
Sign-on bonus $1,500! Rate of pay: $16.25 – $18.25.
Job Duties:
- Transport assigned residents to medical appointments. Ensure all relevant medical information is obtained prior to the appointments and communicated at appointment. Obtain necessary information from medical providers at appointment and thoroughly communicate to appropriate Arc Herkimer staff in a timely manner.
- Implement the goals and objectives of Individual Treatment Plans and facilitate meaningful outcomes as identified by the person supported.
- Record and monitor participant individual progress as indicated by supervisor, program policy and/or regulation.
- Provide direct care and habilitation services, including but not limited to personal hygiene care, toileting, feeding, bathing, lifting, transferring and behavior management techniques as required. Provide oversight for individuals assigned and respond to emergent situations as per program procedures and guidelines.
- Assist Supports & Services programs as needed.
- Perform duties in such a way as to reflect individualization, integration and fulfillment of potential; continually identify and utilize teaching opportunities; acknowledge and respect individual choices and preferences; provide “advocate” duties to include encouraging relationships between persons supported and family/community friends as appropriate.
- Professionally communicating issues of concern which occur while working at a home or on medical appointments immediately and thoroughly to oncoming shift or other staff as appropriate
- Completing all documentation requirements thoroughly and within required timeframe.
- Obtain and retain Arc Herkimer medication administration certification; administer medication competently and safely, ensuring all related documentation
- Undertake such duties, responsibilities and projects as may be required by supervisor and/or director.
Position requirements:
- High School diploma or equivalent.
- Arc Herkimer approvable driver’s license.
- Experience with persons with developmental disabilities preferred.
- Must have accessible telephone.
This position is full time Monday to Friday 8am-4pm with assigned evening coverage on a rotating basis.
