Rate of Pay: $25.78/hour + DOE

Summary

Supports Care Coordination patients and programs. Works with Care Coordinators to provide resource referrals, patient outreach, assessment assistance and coordination of care.

Essential Duties and Responsibilities

  • Contacts patients to provide assistance and coordination of care under the guidance of Care Coordinator.
  • Assists Care Coordinator with assessment of needs, resource coordination and referrals management.
  • Works with Care Coordinators to assist with non-clinical aspects of the Care Coordination program.
  • Provides support to Neuro-Trauma Care Coordinator to contact out of area patients for follow up assessment.
  • Coordinates transportation, housing referrals, food resources and other social determinant needs.
  • Arranges Durable Medical Equipment (DME).
  • Assists with billing of Community Case Management (CCM).
  • Provides assistance with medical record management for specialty referrals.
  • Provides culturally competent patient support.
  • Supports Director with data collection and other projects.
  • Documents interventions in Epic.
  • Demonstrates System Values in performance and behavior.
  • Complies with System policies and procedures.
  • Other duties as may be assigned.

Minimum Education/Experience

No Educational Requirement

6 months to 1 year of relevant experience

Required Licenses/Certifications

Certified Medical Assistant (CMA) or Registered Medical Assistant (RMA) - Upon Hire

BLS: Employee will be enrolled in the Resuscitation Quality Improvement (RQI) Basic Life Support (BLS) Entry or prep Curriculum (depending on their previous BLS Certification) - Within 3 months of hire into role

Other Experience/Qualifications

Required:

  • Knowledge of medical terminology
  • Familiarity with medical documents and medical chart components
  • All certifications must be kept valid and current. Compliance with any future change in testing or certification required by state legislature.
  • A willingness to be trained on-the-job in duties not previously learned
  • Proficiency in EMR (Epic) software
  • Ability to write routine reports/notes and correspondence
  • Strong command of the English language

Preferred:

  • Bi-lingual Spanish
  • Experience as a medical assistant in a primary care setting or care coordination
