POSITION SUMMARY
Work as part of a team with the MAT Nurse to manage all aspects of patient care for incarcerated individuals receiving Medication Assisted Treatment for Opioid Use Disorder and support a recovery-oriented system of care within the facility.
- Initial and ongoing SUD assessment and evaluation
- Individual and group recovery support
- Monitor medication adherence
- Care coordination
- Health promotion
- Comprehensive transitional care and re-entry planning
- Collaborate with community treatment providers and social support services
- Documentation and data collection
BA with previous substance use treatment experience required; LADC or master’s degree with license eligibility preferred.
