We are looking for a talented Nurse Practitioner, who wants to make a difference, by supporting the associates and organization to join our Valor Health Services team.

Responsibilities:

Provide expert, comprehensive, person-centered care to patients with chronic and advanced illnesses in their place of residence.

Build and maintain a panel of primary care patients.

Travel between adult care homes, assisted living facilities, and personal residences for patient visits.

Participate in rotating on-call schedule.

Collaborate and participate in outreach/education in the community with the Valor Community Liaisons.

Qualifications:

Licensed Nurse Practitioner by the State of Arizona

Specialty in geriatrics, palliative care, home health and/or hospice preferred.

Valid AZ Driver’s License and auto insurance and reliable vehicle.

Excellent observation, verbal and written communication skills, problem solving skills, and nursing skills per competency checklist.

The ideal candidate will have a working knowledge of all aspects of primary and palliative care. We are looking for a Nurse Practitioner who will focus on building and maintaining a patient primary care panel. This candidate should inspire trust and treat others with dignity and respect. This person will also be very well organized and have the ability to manage resources efficiently and perform the job duties accurately. He or she must have excellent communication skills, both oral and written. We are also seeking someone who will provide excellent customer service and maintain a high level of confidentiality.
