Job Summary:

Primary function is to oversee and manage telehealth program. LVN would provide administrative and technical support of the telehealth activities including installation, configuration and maintaining hardware of the telehealth units. LVN would provide onsite support services to end users as well as training to agency staff. Work is performed under the general supervision of the administrator and/or Director of Clinical Services. LVN would coordinate care with the interdisciplinary team and patient/caregivers, including contract nursing teams managing alerts as well as daily alerts through telehealth portal. This position will promote patients' potential during their home health episode and collaborates with physician, clinical team and patient in preparation for discharge.

Essential functions:

• 
  Manages and oversees unit inventory, including shipping logistics, cleaning and disinfecting units
• Coordinates with case managers and other clinical team members to identify patients who would benefit from telehealth units
• Contacts potential patients via phone to determine interest and appropriateness for telehealth units
• Requests orders for telehealth units from referring physicians

Qualifications:

• 
  Licensed Vocational Nurse, per the state requirements.
• Prior telehealth experience preferred
• Good communication skills to be able to relate professionally and positively to patients, caregivers, colleagues and physicians and strong clinical documentation skills.
• Ability to operate a personal computer and utilize common office software programs, including reporting tools from telehealth platform

Ability to read, write, and speak the English language.
